Certification of Research Personnel

Certification of New Research Personnel

All personnel involved in animal care, use, or treatment must be certified by the IACUC as qualified to perform their duties. Submit your signed original request for certification to the IACUC c/o Comparative Medicine, MDC 20, telephone 974-9796, fax 974-9432, e-mail CompMed@research.usf.edu by completing the following editable documents .

New Principal and Secondary Investigators: Research personnel with a significant history of work with the species requested, which has been documented by the publication of multiple, peer-reviewed reports, are required to submit a copy of their curriculum vitae to Comparative Medicine (MDC 20). All research personnel must complete a Request for the Orientation and Certification of New Research Personnel Using Animals, linked below.

Health & Safety: All research personnel must complete a Health and Risk Assessment for Employee Safety in the Care and Use of Animals form, linked below, and submit it to Comparative Medicine.

Individuals determined to be at risk (e.g. a pre-existing health condition, research involving unvaccinated or uncharacterized carnivores, pregnant sheep, goats or cattle, or nonhuman primates) as a result of this assessment, must also complete the Comprehensive Health Assessment form, which is linked below. At risk individuals are provided services for documentation/verification of immunization against rabies, rubella, rubeola, varicella, hepatitis B, and diphtheria/tetanus. Documentation of a PPD skin test for tuberculosis, and, if the PPD skin test is positive, documentation of a negative chest radiograph are provided by Health Administration.


Facility Orientation
: Upon completion of a Request for the Orientation and Certification of New Research Personnel Using Animals, and a Health and Risk Assessment form, all new research personnel should contact the local facility manager so that a general orientation, and protocol-specific, and/or species-specific training can be arranged.

During orientation, facility managers and training coordinators will complete the forms linked below:

Field and Wildlife Research Orientation
: Field study participants should receive certification and be added to the respective approved wildlife application. Electronic orientation, rather than face-to-face meeting, is a more relevant and expeditious method of orientation for personnel conducting field studies. Controlled Substances
: Research personnel requesting, possessing, or using any controlled substance in research or teaching must be registered with Comparative Medicine c/o the Surgical Core Manager at MDC 20, phone 974-5001, or fax 974-7552 using a Certification of Research Personnel Using Controlled Substances form, linked below.


Annual Re-Certification of Research Personnel

All personnel involved in animal care, use, or treatment must annually re-evaluate their animal use, declare any change in PI, species, hazardous agents, duties or health condition, and request to be re-certified by the IACUC. To be eligible for re-certification, submit your signed original request to the IACUC c/o Comparative Medicine, MDC 20, telephone (813) 974-9796, fax (813) 974-9432, e-mail CompMed@research.usf.edu prior to July 1st of each year. The University of South Florida has purchased a specific number of ALL training accounts. To be issued/assigned an ALL account, which will allow access to the ALL courses and self-examinations, submit a request to Compmed@research.usf.edu. The request should include your first name, last name, and e-mail address you wish to use for ALL and USF questions and/or contact.

Your request will be processed and you will receive, via e-mail, a "temporary" user name and a "temporary" password for enrolling in ALL. After logging on with the "temporary" information, a change in name and password (suitable to your liking) is suggested.
